Look for a New Form at Tax Time: 1095-C

Additional form required to report employer-provided health insurance

EVANSTON, Ill. --- Due to the Affordable Care Act (ACA), Northwestern University employees will receive an additional Form 1095-C to use when filing tax returns. The purpose of the form is to report employer-provided health insurance coverage.

This year, Northwestern will outsource the processing of W-2 and 1095-C documents to Equifax. In early December, employees will receive an email communication from Northwestern Payroll with a link to an Equifax website. This page will allow users to consent to receive an electronic copy of 2015 W-2 and 1095-C forms for tax reporting. The online consent period will open in the coming weeks, and related communication will link to Equifax, not Northwestern’s FASIS Self-Service system.

By outsourcing W-2 forms to a vendor, past employees will have the ability to access their W-2 and 1095-C forms online, even after they have left Northwestern.

Additionally, Equifax has access to tax programs that employees may use to automatically upload their W-2 information when filing. As in the past, any employee who does not consent to a digital W-2 will have the form mailed to them by Feb. 1, 2016.
